ACCREDITATION FORM

The media interested in obtaining accreditation for the Asymmetry Festival are asked to fill out the accreditation form located below by 31.03.2013. In case of questions, please contact the following email address: aleksandra.gula@firlej.wroc.pl with the obligatory “ACCREDITATION – ASYMMETRY” in the message topic.

At the same time we inform, that we will contact only the selected media.

DESCRIPTION OF PROVISIONS FOR THE FESTIVAL:

Sending in of the accreditation form, is equal to the acceptation of the RULES OF ACCREDITATION GRANTING and an agreement to processing of the given personal data, in accordance with the law act, dated 29.08.1997, on the Protection of Personal Data (Dz.U. Nr 133, poz. 883 [Journal of Laws, no. 133, item 883]) for the needs of the accreditation procedure of the fifth edition of Asymmetry Festival, organized by the Firlej Club in Wrocław. Providing the personal data is not mandatory, but is required for obtaining the accreditation.

RULES OF ACCREDITATION GRANTING

1. Accreditation will only be granted to the media, who send the application form, together with the descriptions of provisions for the festival, to the given e-mail address by March 31st, 2013.

2. The Organizer reserves himself the right to a negative consideration of the application without stating a reason.

3. The Organizer reserves himself the right to answer only to the selected applications.

4. The Organizer will inform, about granting the accreditation, via e-mail, by 15.04.2013 at the latest.

5. Number of accreditations is limited.

6. The accreditations authorize the holder to a free-of-charge entry to the locations where the festival takes place.

7. The rules of recording of the Festival via audio-visual and/or photographic equipment, will be handed down, to the accredited TV photo reporter crew, on the day of the performances at the latest.

8. Accreditations will be granted to maximally two representatives of a given publisher, press agency, radio station or web portal. Exceptions are the Main Media Patrons of the event.

9. In case of violation of the the rules of granting and using the granted accreditation, and the failure to perform all the declared provisions, the Organizer has the right to revoke the granted accreditation.

10. Applications sent after the deadline, incomplete or illegible will not be considered.
